Effective goal setting involves collaboration between the patient, physician and is an important component of appropriate treatment and prognosis. When diagnosed with a long term condition, patients and family care providers will be embarking on a life-long journey. This journey will involve them developing an understanding of their condition, how to manage it and the impact it will have on their lives. The aim of setting treatment goals is for you to live your life to the full and not have your life dominated by the long term condition. It is important that you may want to return to the life you had before you developed the condition. You may feel that you have to readjust or give up normal activities such as socialising, or continuing to work. You may be wondering about what to do or know what is possible.
